What is Secure Dental One? Secure
DentalOne offers you access to high quality affordable dental insurance coverage
for your entire family. Coverage is
provided for preventative, basic and major dental services per insured
person as follows:

First, you meet the $100
Lifetime per person deductible. Next, Secure Dental One pays
a percentage of covered expenses. With the ClassicOne and the
PremierOne dental insurance plan, you can use any dentist.
The Secure Dental One plan also has a Orthodontics option available. |

This plan is offered to all ages, and their
eligible dependents (state requirements for dependent eligibility may
vary). Child only coverage is not available.

When does my coverage start?
Coverage starts on the effective date. The effective date issued will
begin on the first of the month (at 12:00 a.m.) following your
enrollment..

What are my payment options?
You can pay Monthly by or credit card or through auto bank withdrawal .
Quarterly or Annually can be paid by credit card.

What is the calendar year maximum?
The maximum amount payable for all Covered Dental Charges in any
calendar year as shown in the Coverage Schedule above. The Calendar Year
Maximum will apply to each insured person
