Metal Gap 2 insurance includes accident and critical illness benefits to allow you to focus on your number one priority: getting better. Metal Gap 2 insurance provides benefits for covered accidents up to the selected maximum amount, or the actual expense you incur, whichever is less. Critical illness benefits provide a lump-sum payment that is sent directly to you and can be used for anything you choose, including:

 

• Out-of-pocket expenses

• Living expenses

• Lost wages

Fixed Hospital Indemnity Insurance designed to help fill the gaps in your health insurance coverage and reduce out-of-pocket exposure due to a hospitalization.

 

This type of coverage may be the right fit if you are looking for:

Eligibility Independence Hospital Insurance is available to the primary applicant age 18 to 64.5, their spouse age 18 to 64.5, and dependent children under the age of 26. Wellness and Preventive Care Waiting Period The period following your effective date during which no benefits are payable. For all three plans, there is a threemonth waiting period before benefits are payable. There is no waiting period for this benefit in Missouri, North Dakota, New Mexico and Tennessee. The Wellness and Preventive Care benefit is not available in Michigan and New Mexico. Pre-Existing Condition Definition* A condition for which medical advice, diagnosis, care, or treatment was received or recommended during the pre-existing condition limitation period. Pre-Existing Condition Limitation The policy contains a pre-existing condition limitation. This limitation will cease to apply to any covered benefits incurred in connection with a pre-existing condition after the covered person has been continuously covered under the policy for 12 consecutive months1 from the covered person’s effective date for any pre-existing conditions within 12 months2 prior to such effective date. * Definition varies by state 1 6 months in MI, NC, NH, NM, UT 2 6 months in NH, MI, UT

The Accident Medical plans and the Critical illness plans pay you directly. Designed to lower your out of pocket expenses for any other coverage you might have.

 

These plans are a good choice if you have a high deductible plan.

The supplemental plans do not have a limited duration. You can keep your plan as long as needed or cancel at any time.

Your coverage can begin as early as the day after you apply or the first of the month following the date you apply.

No Out-of-Network Penalties

Accidental Medical and Critical Illness plans are considered indemnity plans - meaning they pay a specific dollar amount based on the type of illness or accident. The amount the plan pays has no bearing on what doctor or hospital you use.
